Office Policies

Because the Bolts Pain and Wellness Center is dedicated to your over all health and well being it is necessary that our patients understand a few office polices. This is to insure that clear understanding exist between our patients and staff.

  • Boltz Pain and Wellness Center will not refill prescriptions by telephone or fax for narcotics or pain relievers. You will need to contact our office and schedule an appointment. For emergencies you are encouraged to dial 911 or visit your closest emergency room.
  • To avoid a missed appointment fee of $30.00, contact our office 24 hours prior to your scheduled appointment to make other arrangements.
  • Our office is in compliance with all HIPAA guidelines. Privacy Practices are available to the patient upon request. In summary, our office uses health information about you for treatment, to obtain payment for treatment, for administrative purposes, and to evaluate the care that you receive. This information is contained in your medical record and is the physical property of Boltz Pain Center.
  • Boltz Pain and Wellness Center requires all prescription medical patients to enter into a Narcotic Agreement. The agreement is a guide for healthy pain management between the patient and Dr. Boltz.
  • All prescription medical patients must understand they may be required to submit to random drug screenings. This allows Dr. Boltz the ability to monitor healthy therapeutic levels of medication.
